High up the pitch means in advanced areas - a defensive line playing high up the pitch means one that is (relatively) far away from its own goal.

Deep, medium and high all seem to refer to the defensive line. A team defending "deep" is the opposite to one defending high, i.e. they leave little space between the last defender and the goalkeeper.

If "play out" means "playing the ball out of defence", then it refers to centre backs making offensive passes in order to create attacks, as in the ball comes from out of the defence.
