I think 搬入 is used when a patient is "brought in" such as by ambulance or helicopter, or on a stretcher (for example, from a ward to an operation theater). If a patient shows up on his own, I don't think his arrival is called 搬入.

I would also use "on arrival" for the translation, but what the word means, I think, is that the patient is "brought in".

Perhaps the 入院時 nuance is `being admitted` (the patient formally registers) whereas the semantic difference between 入院＜＝＞搬入 is simply 搬入　is when the individual rolls up/appears/presents (eg. if an unconscious patient arrives by ambulance) but the individual can not actually 入院 (register, show health insurance card, sign waivers, give NOK details etc) but 搬入 (arrival?admitance?presentation?) by definition has occured. Just guessing.
