Abstract
Desmoplastic Small Round Cell Tumour (DSRCT) is a rare, highly aggressive, mesenchymal tumour that arises from the peritoneal cavity. It is commonly seen in adolescent and young adult males and its occurrence in females is uncommon. We are reporting here a rare case of DSRCT in a young woman, which clinically masqueraded as an ovarian malignancy, with metastasis to liver, lung, spleen and peritoneum. The cytologic findings, Histomorphological and immunohistochemical features have been discussed, with a brief review of literature

When a tumor extends beyond the serosal tumor seedling, a peritoneal implant occurs. Most of the peritoneal spread is by the tumor seedling during surgery / biopsy. Peritoneal deposits are often large discrete masses that look like the primary tumor and can extend up to the pelvis [Figure 1] and [Figure 4]. The attenuation and enhancement pattern is similar to that of the primary lesion. A hyperattenuating enhancing mass, with areas of necrosis, hemorrhage, and cystic degeneration are seen. Tumor vessels may be seen. They may appear as large discrete masses / multiple nodules, involving the peritoneal surface and sub- peritoneal spaces, and less commonly it may be a diffuse hypervascular omental / peritoneal caking [Figure 5].

Last week my father had a CT scan due to elevated CEA levels (190) in his blood. The image reader said he has evidence of peritoneal disease with peritoneal caking.

His history is colorectal cancer 6 years ago, he had chemo then went into remission and it came back to his small bowel after two years. He had chemo from January - August last year and his markers went down then sadly went back up again three months later, so he started on more chemo this January - April, which completely has weakened his bone marrow and minerals.

Figure 2 is an axial image at the level of the pelvic inlet of a different patient, who, similarly to the above described case, has ovarian adenocarcinoma with peritoneal disease. However, in comparison to the case above, the peritoneal involvement is focal rather than diffuse. The focal area of peritoneal caking is outlined in pink.

Omental cake refers to infiltration of the omental fat by material of soft-tissue density. The appearances refer to the contiguous omental mass simulating the top of a cake. Masses on the peritoneal surfaces and malignant ascites may also be present.

The greater omentum is basically a peritoneal extension, though not referred to in this case.
