French term

épaississement en \"galette\"

French to English Medical Medical: Health Care
This appears in a CT scan report (chest, abdomen and pelvis) for a patient where there is a suspected peritoneal carcinosis. Here's the context:

Épaississement tumoral du plancher vésical ainsi que de la paroi latérale droite de la vessie jusqu'à
20 mm d'épaisseur environ.
Confirmation d'un épaississement en "galette" du péritoine antérieur sus-ombilical, jusqu'à 30 mm.

I am assuming this refers to the shape or form of the thickening, but I couldn't find any references to "wafer-like" or "cake-like" thickening anywhere - does anyone have any insights for me? Thanks in advance!

Discussion

Susan McDonald (asker) Mar 13, 2019:
Seems in this case it's caking... Many thanks to all who contributed on this question. It was confirmed by the patient (after asking the second specialist) that this is, indeed, caking.
Susan McDonald (asker) Mar 2, 2019:
Think I may need to have sight of the scans Thanks to everyone who has contributed to this particular question. I am translating these docs for a local friend who was diagnosed with pancreatic cancer last year, had chemo, seemed to respond, and now his team have found this peritoneal carcinosis. He is getting the very best of care here, but his daughter wants him to get a second opinion in the UK. Hence the need to have the docs translated. I'll ask to see the scans themselves and see if there's anything marked on them which might clarify matters.

Abstract
Desmoplastic Small Round Cell Tumour (DSRCT) is a rare, highly aggressive, mesenchymal tumour that arises from the peritoneal cavity. It is commonly seen in adolescent and young adult males and its occurrence in females is uncommon. We are reporting here a rare case of DSRCT in a young woman, which clinically masqueraded as an ovarian malignancy, with metastasis to liver, lung, spleen and peritoneum. The cytologic findings, Histomorphological and immunohistochemical features have been discussed, with a brief review of literature

When a tumor extends beyond the serosal tumor seedling, a peritoneal implant occurs. Most of the peritoneal spread is by the tumor seedling during surgery / biopsy. Peritoneal deposits are often large discrete masses that look like the primary tumor and can extend up to the pelvis [Figure 1] and [Figure 4]. The attenuation and enhancement pattern is similar to that of the primary lesion. A hyperattenuating enhancing mass, with areas of necrosis, hemorrhage, and cystic degeneration are seen. Tumor vessels may be seen. They may appear as large discrete masses / multiple nodules, involving the peritoneal surface and sub- peritoneal spaces, and less commonly it may be a diffuse hypervascular omental / peritoneal caking [Figure 5].

Last week my father had a CT scan due to elevated CEA levels (190) in his blood. The image reader said he has evidence of peritoneal disease with peritoneal caking.

His history is colorectal cancer 6 years ago, he had chemo then went into remission and it came back to his small bowel after two years. He had chemo from January - August last year and his markers went down then sadly went back up again three months later, so he started on more chemo this January - April, which completely has weakened his bone marrow and minerals.

Figure 2 is an axial image at the level of the pelvic inlet of a different patient, who, similarly to the above described case, has ovarian adenocarcinoma with peritoneal disease. However, in comparison to the case above, the peritoneal involvement is focal rather than diffuse. The focal area of peritoneal caking is outlined in pink.

There is also widespread reference to omental cake/caking eg https://radiopaedia.org/articles/omental-cake

Omental cake refers to infiltration of the omental fat by material of soft-tissue density. The appearances refer to the contiguous omental mass simulating the top of a cake. Masses on the peritoneal surfaces and malignant ascites may also be present.

The greater omentum is basically a peritoneal extension, though not referred to in this case.

All the above refs deal with cancer

There is a definition of peritoneal caking here - https://www.flashcardmachine.com/peritoneal-abdominal-wallpa...

Peritoneal Metastases - Cellular implantation throughout the peritoneal cavity
Most common primary sources are ovary, stomach and colon
US findings of nodular, sheet-like,irregular configuration of the peritoneal sheath
May be known as peritoneal ‘caking

Therefore I don't think it refers to the shape of the lesion

This French ref refers to "épaississement en galette du grand épiploon" ie omental caking
