Hola nuevamente,

declared before a notary / has made a notarized declaration to that effect

A couple of ideas.

"...in a stable de facto relationship declared before a notary..."
"...in a stable de facto relationship, who has made a notarized declaration to that effect..."
Peer comment(s):

agree Angelo Berbotto : This is how I would render it.

Thanks, Angelo.
agree Adrian MM. : the 'non-existent' English Common Law marriage and supposedly abolished Scots Law 'marriage by habit & repute' come to mind,

(confirmed) by a notarized declaration

This de facto union was confirmed by way of a declaration, probably written, that was notarized by a notary public.

Peer comment(s):

disagree Angelo Berbotto : I am afraid I disagree here. La unión (no el unión) is not confirmed by the notary. The parties make a declaration and this declaration is solemnised in the deed issued by the notary.

Yes, I should have written "la unión" but de facto unions may exist without being declared (see for ex. https://www.notaria19bogota.com/la-union-marital-hecho/). Notarizing gives legal proof of something that pre-exists the declaration.
