I have translated the rest along the lines of "breathing detected, but not examined".

souffle may be murmur (heart). In this case BC could be blocage complet (complete block) but can't work out the R - right? rhythm? but obviously these are English terms

I think (a bit of context would help) that it is a heart examination. A "souffle" is a specific type of abnormal cardiac sound, called a "murmur". BCR would mean "Bruits cardiaques" (heart sounds), called B1 and B2.
So here the doctor heard those sounds (B 1 and B2, together BCR), with a murmur.

I think that CR may stand for something like “cycle respiratoire”, but I’m lost as to the B.
