On the right side it says:

And then below, centered, it says:
"Datos de las personas Contrayentes"
In my mind, I translated it as
"Registration Entity"
"Registration Municipality"
"Marriage Recording Date"
What do you think.. ? Please correct me if I'm wrong.

When deciding between easy and pro, err on the side of pro. Most questions will be pro.

: Yes, as Juan says, "entidad" refers to the "federative entity", which in most cases, other than Mexico City, means which State the registry office is in.

Many other things are subject to registration, such as real estate, patents, trademarks, copyrights, mortgages, etc., with their own Registry Offices. The word "Civil" distinguishes personal status records from these other types of registrations.
