There seems to be a lot of confusion here about this question.

So I will repeat and add some more clarification here (perhaps unnecessary).

'cT3N2M1c' refers to the 'primaire tumor' or 'primaire tumorhaard'.

This 'primaire tumor' or 'primaire tumorhaard' is not mentioned in this phrase:

...bijnier- en hersenmetastasen en 2e (primaire?) longhaard RBK, cT3N2M1c.

but there is a second tumour in the right upper lobe that may be another manifestation of the primary tumour.
Further diagnostic research is necessary.

'2e longhaard' suggests there is also an '1e longhaard’. This ‘1e longhaard’ is obviously the 'primaire tumor' or 'primaire tumorhaard'.
The stage of the [1e] 'primaire tumor / tumorhaard' is: cT3N2M1c.

The ‘c’ in cT3N2M1c means there is clinical (c) evidence of T3 and clinical evidence of M1.

T3N2M1 means, as explained under the other question, that the following has been found as regards the primary tumour:
T3 --> Tumor of any size that directly invades any of the following: chest wall (including superior sulcus tumors), diaphragm, mediastinal pleura, parietal pericardium; or tumor in the main bronchus <2cm distal to the carina, but without involvement of the carina; or associated atelectasis or obstructive pneumonitis of the entire lung (clinical evidence)
N2 --> Metastasis to ipsilateral mediastinal and/or subcarinal lymph node(s)
M1 --> Distant metastasis present (in adrenal(s) and brain in this case) (clinical evidence)

It says (in the other question) that ‘de haard in de RBK is maar beperkt kleiner geworden’.
This suggest that this - possible - second manifestation of the primary tumour is of significant proportions.
One could call it a ‘focus’, perhaps, or a ‘mass’.

My preference was ‘mass’.
