If you need it as a quotation, that's exactly what is said in the Bible: "oculum pro oculo" and the accusative hinges on the forementioned"reddet":
Sin autem quid adversi acciderit, reddet animam pro anima, oculum pro oculo, dentem pro dente, manum pro manu, pedem pro pede,
adustionem pro adustione, vulnus pro vulnere, livorem pro livore.
You could also use the nominative here - and it would be "oculus pro oculo" -, if you need it just as an aphorism.
