A horoscope is sometimes referred to as a "Natal Chart" or a "Birth Chart". This chart shows the positions of the planets relative to the Earth at the moment an individual is born. This chart can tell an astrologer, or the person having their chart done, all about their personality so that the individual can make wiser choices about how they conduct their daily affairs

agree Refugio : I think this is closer to the way the word is used on an everyday basis. It usually refers to published daily or weekly predictions rather than the astral chart itself.

A "Horoscope" is the basic instrument created by an astrologer, for an individual, based on the date, time and place of birth of that individual.
There are a variety of styles used to prepare a horoscope, depending on the branch of astrology. This is a complex science (some prefer not to call it 'science', but that is a different story), and its basic function is to represent the positions of various planets in different zodiac signs (or "houses") at the time the individual was born, and their interplay. From this basic function, along with a host of accessory information, a competent astrologer is able to predict the future life of that individual.

Fortune telling originated in ancient time

Horoscope is a type of fortune telling. Since it is fortune telling, it is (so believed that) closely related to a person's course of life (fate). Though today it is being used to see a person's fate (of a day, a month, a year, as well as entire life from birth to death), in ancient time this was a method to foretell the outcome of war, etc. The combination of positions of twelve zodiacs (Aries, Pisces, Scorpion, etc.), the Sun, the Moon, the Planets, is supposed to affect the State's affair such as war, natural disaster, time of ruler's death, etc. The famous King David's six pointed Star is the most famous Astrological chart, based on the combinations of what I just mentioned. Horoscope also supposed to explain current affairs why such and such have happened to him or her.
