I think, it must Silverman score, in view of the number 5, as other symptoms go only from 0-2. Thanks Sue for the great references

intercostal retractions

Given the other questions, all concerning the Silverman respiratory scale, I guess this one must be intercostal retractions, although I don't know what IS stands for.
