This is an 80-year-old patient who has been hospitalized for decompensated heart failure:
"Nous avons profité de l'hospitalisation pour effectuer un diagnostic éducatif dans le cadre de l'insuffisance cardiaque et intégrons Monsieur X dans notre programme d'éducation thérapeutique."
I found the following definition: "L'OMS définit le diagnostic éducatif comme la « première étape du processus pédagogique. Il s'agit d'un recueil systématique, détaillé et itératif d'informations par le soignant, concernant la situation bioclinique, éducative, psychologique et sociale du patient. Ces informations doivent servir de base pour la construction d'un programme d'éducation thérapeutique personnalisé. » [2]. Many of the translated sites use "Educational diagnosis" but when I search this term on Google it sends me to "education" sites and not medical...

: I don't see how else you could translate this. Joanna's text refers to a "programme d'éducation thérapeutique" and "la première étape du processus pédagogique".

thanks: 1st stage - see the link: (evaluation of flare-up severity, treatments used, etc.), any difficulties encountered, future plans, needs, and so on.
