Do ponto de vista jurídico, não me parece que um simples termo de ciêmcoa e responsabilidade tenha "efeito" de um alvará ou permissão. Interpreto "com efeito de" no sentido de "para emissão/expedição". De qualquer forma, seria bom confirmar com o cliente, pois a terminologia não está clara.

Termo: declaração exarada em processo, entry 6, Novo Dicionario Aurelio da Lingua Portuguesa, Rio de Janeiro. UK: Affidavit; AmE also: Deposition Sworn.

com Efeito de : for the purpose of (cf. para todos os efeitos > to all intents and purposes, Collins), rather than such doc. doubling as a licenc/se in itself.

PS Oliver's Discussion Entry turned my thoughts to a Brewster Hearing in England & Wales Magistrates' Courts and at which the local police can object to an award, grant, regrant or extension of an alcohol licence if there have been too may complaints or fights aka 'pub brawls' e.g. at our hotel restaurant bar at which I used to serve as a 'barista'..
