Many studies have documented the common presence of depressive symptoms among patients seeking treatment for alcoholism (Waldkoetter & Sanders 1997). Depressive symptoms are frequently reported as comorbid factors in such treatment (Meichenbaum 1994), which must be addressed for long-term relapse prevention and for treatment acceptance in the short term. Effective treatment programs for substance abuse (SA) are no longer dependent upon the twelve-step Alcoholics Anonymous (AA) model as the only treatment modality. More often SA programs have now moved to models emphasizing cognitive-behavioral approaches and to somewhat decreased reliance on the AA model except as an ongoing support function after formal treatment. Such programs recognize that cognitive thought patterns have contributed to and reinforced dysfunctional lifestyles and life-long behavioral patterns leading to alcohol dependence or abuse.
Modern programs frequently are patterned after models such as the American Society of Addictions Medicine’s (ASAM 1991) that emphasize individualized, flexible treatment with specific criteria guiding level of care and length of services. Programs using the ASAM model to determine levels of entry and of care in treatment have found that ASAM placement criteria result in the patients being placed at less intense treatment levels than do programs with fixed entrance and levels of care. The placement at lesser levels of intensity has allowed many such programs to substantially reduce the cost of treatment.

W wielu badaniach udokumentowano typową obecność symptomów depresji u pacjentów zgłaszających się na leczenie alkoholizmu (Waldkoetter i Sanders, 1997). Stwierdzono, iż symptomy depresji często towarzyszą chorobie alkoholowej (Meichenbaum, 1994) i muszą być leczone, w dłuższym okresie czasu - aby zapobiec nawrotom choroby, a w krótkim okresie – aby terapia była skuteczna. Model dwunastu kroków Anonimowych Alkoholików (AA) nie jest już wykorzystywany jako jedyna metoda w efektywnych programach leczenia uzależnień (Substance Abuse – SA). Obecne programy SA znacznie częściej funkcjonują w oparciu o modele preferujące podejście kognitywno-behawioralne, nieco mniej zależne od modelu AA, z wyjątkiem funkcji stałego wsparcia po okresie formalnego leczenia. W programach tych uznaje się, iż kognitywne wzorce myślowe przyczyniają się i potęgują dysfunkcyjne style życia i wzorce zachowania, prowadzące do nadużywania lub uzależnienia od alkoholu. Nowoczesne programy często oparte są na takich modelach, jak American Society of Addictions Medicine’s (ASAM, 1991), w których nacisk kładziony jest na zindywidualizowane, elastyczne leczenie, gdzie specyficzne kryteria decydują o poziomie (kategorii) intensywności opieki i czasie jej trwania. W programach wykorzystujących model ASAM do określenie początkowego poziomu opieki oraz poziomu stosowanego w trakcie leczenia stwierdzono, że kryteria ASAM prowadzą do stosowania mniej intensywnych poziomów leczenia niż w programach o ustalonych (niezmiennych) poziomach. Umieszczanie pacjentów na niższych poziomach intensywności opieki pozwoliło na istotne obniżenie kosztów leczenia w wielu takich programach.
