Allergic reactions to metallic orthopaedic implant materials are usually of Type IV delayed-type hypersensitivity (DTH)38,39, the most common sensitizers being Ni, Co and Cr40. Titanium hypersensitivity reactions are relatively rare although they are reported in the literature. Much of the evidence is based on clinical case reports.
For example, Lalor at al41 looked at tissues from five patients who had undergone revision surgery for failed hip replacements and found high levels of particulate titanium. The presence of abundant macrophages and T-lymphocytes suggested sensitisation to titanium. Skin patch testing with Ti salts was negative in all patients although two tested positive to a titanium-containing ointment. Van Opstal and Verheyden42 report the case of a woman who developed dermatitis following knee replacement with a Ti6Al4V tibial baseplate and whose symptoms resolved completely when this was replaced by a customised oxidised zirconium (no Ti) component.
Other case reports relate to use of titanium alloy in osteosynthesis devices. Thomas et al43 report a case of a patient who developed localised eczema and impaired fracture healing after receiving titanium-based miniplate and screws to treat a metacarpal fracture. Patch testing showed no reaction to Ti or other metals but in the lymphocyte transformation test (LTT) there was a marked reaction to titanium. After removal of the Ti implants the eczema cleared and the fracture healed; a diagnosis of titanium allergy was made.
Another suspected case of hypersensitivity was reported by Goto et al44, in this case to a Ti6Al4V suture anchor in the shoulder. The patient experienced erythema and itching after receiving the anchor. Patch tests for Ti, V and Al gave negative reactions but the anchor was removed. It was found that the rotator cuff was well healed and surrounding bone had not eroded and did not show signs of inflammatory response. However, the erythema and itching disappeared immediately after the anchor was removed.
Nawaz and Wall45 reported a case of drug rash with eosinophilia and systemic symptoms (DRESS) in a patient who had received titanium prosthesis for a spine fracture. However, hypersensitivity to drugs could not be eliminated. The DRESS syndrome resolved without removal of the titanium implant.
Outside of orthopaedics there are other cases reported relating to dental implants (Egusa et al46, du Preez et al47), titanium bars used in the Nuss procedure for pectus excavatum (Sakamoto et al48), ear piercing (High et al49) and cranioplasty (Hettige and Norris50), the latter leading to mortality due to allergic reaction of the brain.
As well as these case reports there have been a number of larger studies of titanium and/or metal sensitivity patients receiving orthopaedic or dental implants. In an early study, Elves et al51 looked for sensitivity to Cr, Co, Ni, Mo, V and Ti by patch testing in fifty patients who had received total joint replacements. They found 38% were sensitive to one or more of these metals, mostly nickel and cobalt. None were found to be sensitive to titanium although there was one reaction to vanadium. They concluded that metal-on-metal joint replacements may sensitise the patient to metals in the prosthesis and that although there was a high incidence of prosthesis failure in metal-sensitive patients it was uncertain whether the loosening caused the sensitisation or vice versa.

En un estudio previo, Elves et al.51 buscaron sensibilidad al Cr, Co, Ni, Mo, V y Ti usando pruebas del parche en cincuenta pacientes que habían recibido remplazos totales de articulaciones. Encontraron que el 38% eran sensibles a uno o más de estos metales, mayormente al níquel y al cobalto. No se encontró ninguno que fuera sensible al titanio, aunque hubo uno que reaccionó al vanadio.
