The Greek government boasts on Facebook of the lowest statistics in years. Indeed, in 2019, about 60,000 refugees reached Greece by the sea and in August less than 9 thousand (according to UNHCR).
Last year, about 15,000 arrived by land, in 2020 the total is reported to be 3,000 so far. Has the help on the spot been effective? Are the persecutions over? According to various activists, new methods of persecution have started – here in Europe.
The eyes of the whole world are on the Lesbos, where the overcrowded refugee camp burned down, leaving 13,000 people without shelter. Non-governmental organizations present in Lesbos have reported about people trapped for several days between police blockades, left on the road without access to food, water, shelter and information on what to do next.
“Police used tear gas on refugees, limiting the volunteer access, but they were still allowing right-wing groups to act. The level of dehumanization of refugees is appalling " - reports OKO journalist Paweł Kołodziej from Legal Center Lesvos.
There are voices criticising the regime, even internally. “There is no migration crisis in the European Union now. But unfortunately, migrants remain in a state of the crisis” says Ylva Johansson, EU commissioner for home affairs. Although he points out that the conditions in the camp are the responsibility of Greece, he admits that one of the sources of the problem is the failure of the EU asylum reform.
The European Union, however, has far worse things on its conscience than the current crisis in Lesvos. "I am very concerned about reports about migrants push-backs at the borders so that they cannot even apply for asylum" says Johannson, referring to a practice that NGOs have been warning about for years.
While the European Union has been debating a solution to the refugee crisis for years, overburdened border states are trying to cope with the influx of refugees on their own. Border violence activists report on an unprecedented scale the number of illegal push backs by services at EU borders. Their brutality has increased rapidly since March.
In March, photos and videos of people pushing through the Greek-Turkish border were released to the media. President Erdoğan decided to show his strength. By breaking the pact with the European Union, according to which he is to keep refugees in Turkey, he unilaterally opened the borders. Europe trembled. "Biological War? Turkey is trying to send coronavirus infected migrants to Greece” reported severa Greek newspapers. Reports of Greek police brutality, tear gas, firearms and violations of the right to apply for asylum fell into a political vacuum. “This border is not only a Greek but also a European border. I thank Greece for being our shield these days, ”said the President of the European Commission, Ursula von der Leyen.
On the other side of the “shield”
“Some were at the border of their own free will, others were just brought there. Refugees from various parts of Turkey were brought in and held imprisoned at the border for a month,” commented Natalie Gruber from the Austrian NGO Josoor, which works with the Border Violence Monitoring Network: “We tried to provide them with the most necessary items, but the Turkish police prevented us from helping on a larger scale. They did everything to attempt to force people to try and cross the border. "
In late March, those who remained on the Turkish side were deported and dumped at random locations in the middle of lockdown Turkey. “Most of them were also handed deportation documents. The reason given was an attempt to cross the border illegally. Some people were taken to the coast and encouraged to try to go to Greece, ”says Gruber.
"We are cards and they play us," commented the father of one of the Syrian families imprisoned on the border in Pazarkule.
Erdoğan's game
Turkish services are still eager to push people to Greece. “If you try to get to Bulgaria or Italy, they stop you. Are you going to Greece? Sometimes they will even advise you on the best place to go on any one particular day.” says Gruber.
Turkey and Greece share the Mediterranean Sea with its oil and gas reserves. The conflict over who will take them over has been going on for years and Erdoğan is doing everything to win it. And refugees flee to Europe because they have no choice. They may be deported from Turkey at any time.
Turkey signed the Geneva Convention relating to the status of refugees, but adopted a territorial restriction - it accepts asylum applications only from citizens of European countries.
In 2014, an exception was introduced for Syrians, granting them the status of "guests" and temporary protection. But public approval for their presence is declining, and Erdoğan, with weakening political support, is reluctant to admit that 3.5 million people have come to Turkey, but will have nowhere to return to for many years. There are more and more deportations.

“We've heard a lot about the arrest of Afghans since June. We do not know how many there are and what is happening to them, because they take their phones away when they are arrested. All we can see is that hundreds of Afghans have simply disappeared in the past months,” says Gruber.
Syrians are also not safe. As late as 2019, Amnesty International reported that 315 "voluntary returns" had been the result of frauds and threats. Gruber hears more and more often about the incorporation of Syrians into the Turkish army and sending them to the front in Libya. The government in Tripoli, supported by Erdoğan in the civil war, is keen to support the idea of creating a new sea border between Turkey, Greece and Libya.
Desperate people keep fleeing to Greece. But politics is already there before them.

Last call from Evros
The main crossing routes from Turkey to Greece are either on the Evros River or the Aegean Sea. Evros has become difficult lately. Refugees are caught at the border or beyond, even after several days of walking. By whom? Sometimes by the police, and sometimes by people in balaclavas about whom nothing is yet known. “There is no official information about who they are. We only know that they cooperate with the police, because they usually hand over caught people to them,” Gruber mentions.
According to EU law, anyone on the Greek side of the border can apply for asylum, and tell their story, which will be verified by the relevant institutions. The reality is different. “The government services take their cell phones, money, documents, clothes and shoes. They keep them for days in some gloomy warehouse. They bring more. When they have 60, sometimes 100 people, they take them to the river. Sometimes in military vans,” says Gruber.
In some instances, they put refugees on boats and illegally deport them to Turkey. The carriers were often other refugees who had struck a deal with the Greek police. "For a month of work, they got a document that allowed them to travel through Greece to the Albanian border." Gruber says.

Increasingly often there are no boats. “The Evros is a dangerous river, it has very strong currents, but sometimes, when the water level is lower, it can be crossed by flowing in some places. We are hearing increasingly often that people are being forced to cross the river,” Gruber mentions.
The degree of cruelty is constantly increasing. "A month and a half ago, we heard about a whole group who had their hands tied behind their backs and thrown into the river to swim to the other side." Gruber also has mentioned evidence of cases of refugees being abandoned on islands in the middle of the river.
“At the end of May, the mother of a Pakistani boy, Faysal Rahmouni, came to see us. She said her son had called her from the island. Others went to the mainland, but he and the two Moroccans could not swim. He sat there for almost a day, he was desperate. " After that, he never called again. He was 16 years old.
The bodies found on the Greek side of Evros back then did not match the description. The mother is still looking for the boy. There are more cases. “Yesterday I heard the story of a group of Moroccans who were told to jump into the water in the middle of Evros and swim to the other side by the border guards. Many could not, two disappeared under the water. Two weeks have passed but they haven't been able to tell the family” says Gruber.
“We don't know exactly how many people are killed trying to get through. The investigating judge who identifies the bodies found in Evros estimates that 1,500 people have been found in the last 10 years. But this is only the Greek side of the border.”
The appearances are still kept up on Evros. Illegal deportations take place at night. There is more freedom at sea. Coastguard activities in the middle of the day in good weather can be seen from Lesvos and the Turkish coast.
Abandoned at Sea
The migration route is changing rapidly in response to ever-changing border defense strategies. Contrary to the popular political strategy of "let's stop accepting, they will stop coming", people still have more to lose where they came from. When Evros is too difficult and therefore too expensive (fivefold price increase in June), people choose the sea. It is not known where it is currently worse.
"We are saved," said Winny, a 35-year-old activist from Uganda, when she saw a boat with a Greek flag arriving. She showed that there were small children on board. On her hands she had her 4-month old child. In response, a stream from a water cannon struck them from a Greek coast guard boat. It hit the child who fell out of Winnie's hands and was thrown overboard. At the last moment, the brother sitting next to them caught the baby.
Many reports mention jabbing with a stick with a hook at the end, spraying suffocating and blinding powder, shots at refugee boats, which are even recorded on video. Here, too, masked men often appear, to whom the Greek services do not officially acknowledge their presence.
According to Gruber, a new strategy has been in force at sea since April. The guards not only block the boats from reaching the shore.
"They take passengers on their boat, sink the dinghies, and then put 30 people in an inflatable life raft scheduled for 15. The rafts are then dumped at the Turkish sea." Such rafts are increasingly used to expel people who have already arrived on Greek land.
Official data shows 923 people drowned crossing the Mediterranean Sea in 2019. We will never know the real number of how many die, because many disappear under water unnoticed.

Deportation at any cost
Until recently, the terrible uncertainty and sense of danger ended on the Greek side of the border. It was possible to rest, apply for asylum, get shelter - even a piece of tent in an overcrowded camp. Now, round-ups and illegal deportations extend deep inland to the Albanian border. It's easy to disappear.
“We have six cases of people who obtained refugee status in Germany or Austria and visited friends or family in Greece. The police identified them, took their documents and illegally deported them together with others. They haven't been able to get out of Turkey for years."
There is now less safe space, you have to move quickly. "Somewhere they will welcome us, somewhere it will be safe," says Hosein, whom we meet at the Albanian border with a family of 10. Two children sleep in prams, the other two are walking alongside. Rasha is three months pregnant, her companion is six months pregnant.
They are starting the Balkan route, which is also becoming more dangerous. Activists have been hearing reports of police beatings, dog rats, and taking shoes and clothes during winter crossings. BVMN devoted a separate report to the violence on the Balkan route. It pays off to pursue refugees. “They often have savings accumulated over decades with them. Even tens of thousands of euros. We don't believe that all this goes to the state treasury, ”says Gruber.

The European Union sees nothing
Frontex, the European Border and Coast Guard Agency, has been present on Evros for years, and also patrols the sea. But they failed to identify "serious incidents" of human rights violations. It also does not confirm illegal deportations on the Balkan route, although - according to Gruber's information - the services of various countries seem to cooperate by transferring refugees at the borders.
The material about the illegal activities of the Greek coastguard was published, among others, by The New York Times. They also wrote about illegal places of detention where refugees are secretly held before being pushed to the border. The Guardian, in turn, reported on cooperation between the European navy and the Libyan border guards in the Mediterranean Sea. The Danish Frontex crew patrolling the sea on the Turkish-Greek border has admitted refusing to participate in the illegal repulsion of refugees. The practice was also noticed by the German supply ship "Berlin". There are reports of Frontex being involved in illegal deportations at the Albanian border. Spiegel also wrote about the pushbacks on Evros.
Commissioner Johnasson declared that she would "consider introducing a new mechanism to monitor whether EU countries respect fundamental human rights at their borders".
And if they don't, what then?
In an interview with OKO.press, a member of the European Commission stated that the journalistic materials, although disturbing, were not sufficient evidence. Anyway, neither Frontex nor the European Commission have any possibility of investigation into this matter. Greek Prime Minister Kiriakos Mitsotakis has denied that his country is illegally pushing away refugees. He called the reports fake news spread by smugglers whose business are supposedly spoilt by Greek actions. He also described Greece's migration policy as "strict but fair". "Greece cannot be the gateway to Europe," explained the Greek Minister for Migration.

Che le persecuzioni sono finite? Secondo vari attivisti, si sono fatti strada nuovi metodi di persecuzione - qui in Europa.
Gli occhi di tutto il mondo sono rivolti a Lesbo, dove il sovraffollato campo profughi è bruciato, lasciando 13.000 persone senza un tetto. Le organizzazioni non governative presenti a Lesbo hanno riferito di persone intrappolate per diversi giorni tra i blocchi di polizia, lasciate per strada senza accesso a cibo, acqua, riparo o informazioni su che cosa fare dopo la tragedia.
"La polizia ha usato gas lacrimogeni sui rifugiati, limitando l'aiuto dei volontari, mentre ancora permettevano ai gruppi di destra di accedere all’area. Il livello di disumanizzazione dei rifugiati è spaventoso " - riferisce Paweł Kołodziej, giornalista OKO, dal Centro Legale di Lesbo.
Ci sono voci che criticano il regime, anche internamente. "Attualmente, non c'è alcuna crisi migratoria nell'Unione Europea ma, purtroppo, i migranti restano in uno stato di crisi " dice Ylva Johansson, commissario europeo per gli affari interni. Pur sottolineando che le condizioni del campo sono responsabilità della Grecia, Ylva Johansson ammette che una delle fonti del problema è il fallimento della riforma delle norme europee in materia di asilo.
L' Unione europea, tuttavia, ha sulla coscienza cose ben peggiori dell'attuale crisi di Lesbo. "Sono molto allarmato per le notizie sui respingimenti dei migranti che avvengono alle frontiere in modo che non possano nemmeno chiedere asilo" dice Johannson, riferendosi a una pratica sulle quale le ONG da anni lanciano l'allarme.
Mentre l'Unione europea ha discusso per anni una soluzione alla crisi dei rifugiati, gli stati di frontiera stanno cercando di far fronte all'afflusso sempre crescente in maniera autonoma. Gli attivisti di Border Violence riferiscono che il numero di respingimenti illegali da parte dei servizi alle frontiere dell'Unione Europea ha raggiunto livelli senza precedenti e quindi allarmanti, la cui brutalità è aumentata rapidamente da marzo.
Proprio a marzo, sono stati divulgati ai media numerose foto e video di persone che premevano per attraversare il confine greco-turco. È stato allora che il presidente turco Erdoğan ha deciso di mostrare la sua forza. Rompendo il patto con l'Unione Europea, secondo il quale deve prendersi cura dei rifugiati in Turchia, ha aperto unilateralmente le frontiere. L'Europa ha tremato. "L'inizio di una guerra biologica? La Turchia sta cercando di mandare migranti infetti da coronavirus in Grecia” hanno riportato diversi giornali greci. Le testimonianze sulla brutalità della polizia greca e l'uso di gas lacrimogeni, armi da fuoco e violazioni del diritto di richiedere asilo sono rimasti inascoltati da parte della classe politica. "Questo confine non è solo un confine greco ma anche europeo. Ringrazio la Grecia per essersi comportata come nostro scudo in questi giorni", ha detto il Presidente della Commissione Europea, Ursula von der Leyen.
Dall'altra parte dello “scudo”
"Alcuni si trovavano al confine di loro spontanea volontà, altri vi erano stati portati con la forza. Diversi rifugiati provenienti da varie parti della Turchia sono stati trasferiti al confine e lì tenuti imprigionati per un mese”, ha commentato Natalie Gruber della ONG austriaca Josoor, che lavora con Border Violence Monitoring Network: “Abbiamo cercato di fornire loro beni di prima necessità ma la polizia turca ci ha impedito di aiutare su larga scala. Hanno fatto di tutto per costringere le persone ad attraversare il confine. "
Verso la fine di marzo, coloro che sono rimasti nel territorio di frontiera turco sono stati deportati e scaricati in luoghi completamente casuali nel bel mezzo del confinamento in Turchia. Il gioco di Erdoğan
La polizia turca è ancora desiderosa di forzare i rifugiati ad entrare in Grecia. "Se cerchi di attraversare il confine per arrivare in Bulgaria o in Italia, ti fermano. Stai andando in Grecia? A volte ti consigliano persino il posto migliore dove andare a seconda del giorno." dice Gruber.
Turchia e Grecia condividono il Mar Mediterraneo con le sue riserve di petrolio e gas. Il conflitto su chi conquisterà l'uso delle preziose risorse è in corso da anni e Erdoğan sta facendo di tutto per vincerlo. Nel frattempo i rifugiati fuggono in Europa perché non hanno altra scelta. Possono essere espulsi dalla Turchia in qualsiasi momento.
La Turchia ha firmato la Convenzione di Ginevra relativa allo status dei rifugiati ma ha adottato una restrizione territoriale - accetta le domande di asilo solo da parte di cittadini dei paesi europei.
Nel 2014 è stata introdotta un'eccezione per i siriani, concedendo loro lo status di "ospiti" e la conseguente protezione temporanea. Purtroppo l’opinione pubblica approva sempre meno la loro presenza e Erdoğan, con un supporto politico che si sta sempre più indebolendo, è riluttante ad ammettere che 3,5 milioni di persone sono venute in Turchia ma non avranno un posto dove tornare per molti anni. Ci sono via via sempre più deportazioni.
"Dal mese di giugno, abbiamo sentito molto parlare dell'arresto di Afghani. Non sappiamo quanti ne siano stati arrestati e cosa stia succedendo loro perché i loro telefoni vengono confiscati al momento dell'arresto. Tutto ciò che possiamo notare è che centinaia di Afghani sono semplicemente scomparsi negli ultimi mesi", afferma Gruber.
Anche i siriani non sono al sicuro. Già nel 2019, Amnesty International aveva riferito che 315 "rimpatri volontari" erano in realtà il risultato di truffe e minacce. Gruber sente sempre più spesso parlare dell'ingresso di siriani nell'esercito turco e del loro invio al fronte al confine con la Libia. Il governo di Tripoli, sostenuto da Erdoğan nella guerra civile, è pronto ad appoggiare l'idea della creazione di un nuovo confine marittimo tra Turchia, Grecia e Libia. Gente disperata continua a fuggire in Grecia. Ma la politica le ha già tagliato la strada.
Ultima chiamata da Evros
Le principali vie di passaggio dalla Turchia alla Grecia sono il fiume Evros e il Mar Egeo. Attraversare il fiume Evros è diventato difficile ultimamente. I rifugiati vengono catturati al confine o oltre, anche dopo diversi giorni di cammino. Ma da chi? A volte dalla polizia ma, altre, si tratta di persone con passamontagna di cui non si sa ancora nulla. "Non ci sono informazioni ufficiali su chi siano realmente. Sappiamo solo che collaborano con la polizia, perché di solito consegnano loro le persone catturate”, afferma Gruber.
Secondo il diritto dell'UE, chiunque si trovi sul lato greco del confine può chiedere asilo e raccontare la propria storia, che sarà poi verificata dalle istituzioni competenti. La realtà è ben diversa. "I servizi governativi sequestrano ai migranti telefoni cellulari, soldi, documenti, vestiti e scarpe. Li tengono per giorni in qualche tetro magazzino dove continuano a portare un maggior numero di rifugiati. Quando il gruppo raggiunge 60, a volte 100 persone, lo portano al fiume, a volte all’interno di furgoni militari", dice Gruber.
In alcuni casi, i rifugiati sono fatti salire su barche e deportati illegalmente in Turchia. Spesse volte, i trasportatori erano essi stessi rifugiati che hanno stretto un accordo con la polizia greca. "Per un mese di lavoro, hanno ottenuto un documento che ha permesso loro di viaggiare attraverso la Grecia fino al confine albanese." afferma ancora Gruber. Sempre con più frequenza, non ci sono barche a disposizione. "L'Evros è un fiume pericoloso che ha correnti molto forti, ma, a volte, quando il livello dell'acqua scende, può essere attraversato scorrendo in alcuni punti. Siamo sempre più spesso avvisati che le persone vengono costrette ad attraversare il fiume”, cita Gruber.
Il grado di crudeltà è in costante aumento. "Un mese e mezzo fa, abbiamo sentito parlare di un intero gruppo che, con le mani legate dietro la schiena, era stato gettato nel fiume per nuotare dall'altra parte." Gruber ha anche menzionato prove di casi di rifugiati abbandonati su isole in mezzo al fiume.
"Alla fine di maggio, la madre di un ragazzo pakistano, Faysal Rahmouni, è venuta a trovarci. Ha detto che suo figlio l'aveva chiamata dall'isola. Altri rifugiati avevano raggiunto la terraferma ma lui e due marocchini non sapevano nuotare. È rimasto lì per quasi un giorno, disperato. "Dopodiché non ha mai chiamato di nuovo. Aveva 16 anni.
Allora, i corpi trovati sul lato greco del fiume Evros non corrispondevano alla descrizione e la madre sta ancora cercando suo figlio. Ci sono molti casi simili a quest’ultimo. "Ieri ho sentito la storia di un gruppo di marocchini a cui è stato ordinato dalle guardie di frontiera di saltare in acqua e di nuotare dall'altra parte. Molti non sapevano nuotare e scomparvero nelle correnti del fiume. Sono passate due settimane ma non sono ancora stati in grado di dirlo alle famiglie” dice Gruber.
“Non sappiamo esattamente quante persone vengono uccise cercando di attraversare il fiume. Il giudice istruttore che identifica i corpi trovati a Evros stima che siano stati ritrovati 1.500 cadaveri negli ultimi 10 anni. E questo è solo il lato greco del confine.”
Le apparenze sono mantenute durante il giorno lungo il fiume Evros. Le deportazioni illegali avvengono di notte.
C'è più libertà in mare. Le attività portate avanti nel bel mezzo della giornata e con il bel tempo dalla guardia costiera possono essere osservate sia da Lesbo che dalla costa turca.
Abbandonati in mare
La rotta migratoria sta cambiando rapidamente in risposta alle strategie di difesa delle frontiere che sono in continua evoluzione. Contrariamente alla strategia politica diffusa del "se smettiamo di accettare, smetteranno di venire", le persone hanno ancora molto da perdere da dove vengono. Quando attraversare il fiume Evros diventa troppo difficile e quindi costoso (i prezzi sono aumentati cinque volte solo nel mese di giugno), la gente sceglie il mare. Non sappiamo dove sia peggio. "Siamo salvi", ha detto Winny, un'attivista Ugandese di 35 anni, quando ha visto muoversi nella sua direzione una barca con bandiera greca. Ha fatto vedere che c'erano bambini piccoli a bordo. Deportazione ad ogni costo
Fino a poco tempo fa, era possibile lasciare dietro di sé la terribile incertezza e il senso di pericolo una volta arrivati sul lato greco del confine. I migranti potevano riposare, chiedere asilo, ottenere un riparo - anche solo un angolo di tenda in un campo sovraffollato. Ora, rastrellamenti e deportazioni illegali si verificano nell'entroterra fino al confine albanese. È facile sparire.
"Abbiamo sei casi di persone che hanno ottenuto lo status di rifugiati in Germania o in Austria e hanno visitato amici o familiari in Grecia. La polizia li ha identificati, ha preso i loro documenti e li ha deportati illegalmente insieme agli altri. Non sono stati in grado di fuggire dalla Turchia per anni."
Gli spazi sicuri sono pochi e i migranti devono muoversi velocemente. Sono in cammino per la rotta balcanica, che sta diventando sempre più pericolosa. L'Unione europea non vede nulla
Frontex, L'Agenzia europea della guardia di frontiera e costiera, è presente da anni sul fiume Evros oltre a dedicarsi alla pattuglia del mare. Source text - English The U.S. Food and Drug Administration has approved Sunovion’s Kynmobi (apomorphine hydrochloride) as an on-demand sublingual treatment for off episodes, or times when medication wears off, in people with Parkinson’s disease.
The medication, formerly known as APL-130277, is an apomorphine film that is placed under-the-tongue (sublingual administration) when patients start experiencing a worsening of their symptoms. It can be taken up to five times a day, at doses ranging from 10 mg to 30 mg.
Sunovion expects it to become available to patients in the U.S. by September.
“Today’s approval of Kynmobi advances treatment options for people with Parkinson’s disease who experience OFF episodes and the associated disruption of everyday activities,” Antony Loebel, MD, president and CEO at Sunovion, said in a press release.
“We are pleased to offer the Parkinson’s disease community a novel treatment option that we believe offers a convenient way for patients to rapidly improve impaired movements and better control their motor symptoms when they need it,” Loebel added.
Levodopa is considered the gold standard for Parkinson’s treatment. But several years after starting the medication, most patients begin experiencing fluctuations in their motor symptoms caused by a faster wearing off of the treatment’s effects. These “off” episodes can happen at any time of the day, and most patients experience more than one episode each day.
Kynmobi’s active ingredient, apomorphine, can cross the blood-brain barrier, a semipermeable membrane that protects the brain from the external environment, and mimic the effects of dopamine in the brain. As such, it can counteract the loss of dopamine-producing neurons in the brain, a hallmark of Parkinson’s.
Apokyn, by US World Meds, is an approved apomorphine injection treatment for Parkinson’s patients experiencing off episodes. Its efficacy in easing motor symptoms is established, but it can pose significant challenges to patients, such as the need for an under-the-skin injection, an initial dose titration that should be supervised in a clinic, and common side effects such as nausea and injection site complications. These challenges are thought to have limited its use.
Kynmobi is expected to provide an easier mode of administration and to be eliminate more slowly from the body, helping to ease the feeling of nausea caused by abrupt reductions in apomorphine blood levels.
“Several years after a person is diagnosed with Parkinson’s disease they may notice problems such as having trouble getting out of bed in the morning or having difficulty getting out of a chair, or that they feel frozen while trying to walk as the effect of their maintenance medication diminishes,” said Stewart Factor, professor of neurology at Emory University School of Medicine.
“The approval of Kynmobi affords health care providers with a needed option that can be added to their patients’ medication regimen to adequately address OFF episodes as their Parkinson’s disease progresses,” Factor added.
Kynmobi’s approval was based on data from a Phase 3 study (NCT02469090), in which the oral medication was compared to placebo as an on-demand treatment of motor symptoms during off periods.
The trial included 109 patients who had at least two hours of total off periods per day, including well-defined morning off episodes, despite being responsive to levodopa treatment. In an initial open-label phase, all enrolled were given increasing doses of Kynmobi (10−35 mg) until an optimal dose was identified.
Patients were then randomly assigned to either Kynmobi or placebo, taken to treat up to five off episodes throughout a day, for 12 weeks. All continued to receive their stable anti-parkinsonian medications.
The trial’s main goal was improvements in motor symptoms — defined as changes on the Movement Disorder Society Unified Parkinson’s Disease Rating Scale (MDS-UPDRS) Part III — from before dosing to 30 minutes after dosing at week 12.
Results showed that patients using Kynmobi experienced an 11.1 point reduction in their MDS-UPDRS scores at week 12, while those on placebo showed reductions of 3.5 points. This 7.6-point difference between both groups was significant, and clinical improvements were seen as early as 15 minutes after dosing, and persisted for up to 90 minutes.
Kynmobi also helped more patients (31%) achieve full control of their motor symptoms — a full “on” response — within 30 minutes at week 12, compared with 14% of those given a placebo.
The treatment was generally well-tolerated, with most treatment-related side effects being mild to moderate, and reversible after its use was stopped. The most common were nausea, sleepiness, and dizziness. One person with known cardiac risk factors who was treated with Kynmobi died due to heart failure.
“We know from our research and discussions with the Parkinson’s community that OFF episodes can significantly disrupt a patient’s daily life,” said Todd Sherer, PhD, CEO, The Michael J. Fox Foundation for Parkinson’s Research. “The Foundation supported early clinical development of sublingual apomorphine, and this approval brings an important new treatment option for people with PD [Parkinson’s disease] who experience OFF [periods].”
