Once you leave the medical facility, call your building nurse at the AP clinic number listed above to give an update about
your health status. The nurse will give you return to work
instructions.
You will need documentation from the medical facility stating the date you are cleared to return to work.
Describe your job to the Treating Provider. If restrictions or accommodations are needed:
o The Provider must document the specifics of the restriction or accommodation.
“Light duty” is not specific enough.
o The restriction must have a start and end date
(Or next follow up date with the provider).
If you are unable to reach your AP clinic nurse by phone, take your documentation from the medical facility visit to your AP medical clinic nurse.
