A syphilis test is required for male and female. A rubella test is required for female only.
This study selected six estimate equations of residual volume for males and females respectively, as shown in Table 1
for male and female and for males and females. Which is correct?

In your SECOND example, only the plural is really acceptable.
However, in the FIRST example you quote, the singular from IS acceptable in this kind of medical context. When talking in general terms about test subjects, the singular form is sometimes used in this way --- it's as if it was being used like an adjective describing a 'missing' noun, for example: 'male and female [patients]'

making sure you maintain the plural if a general intent is present in the instruction or narrative. Note that by coincidence of alphabetical order "hombres" in this case comes before "mujeres", otherwise keep an eye out for putting gender nomenclature in alphabetical order to avoid any disrimination.
