I am having a problem understanding the following sentence, more specifically "fixant", and in light of the following "change l'appellation en":

Le Décret N°95-253/P-RM du 30 Juin 1995, fixant Organisation et Attributions de l’Etat – Major Général des Armées, des Etats – Majors et des Services rattachés, change l’appellation en Direction des Services de Santé des Armées (DSSA).

it can sometimes be "setting out" but "setting out" is usually used for "rules"

It is simply saying that decree.... establishing..... (written as a reminder of what the decree is about) changes the name to Direction...

I understand the meaning of the word fixant, however in the context of the full sentence it becomes obscure :)

Something along these lines:
Order [or Decree, or leave in French] 95-253/P-RM dated 30 June 1995, which lays down the structure, responsibilities and powers of the Defence Staff [for UK readership] and the other Staffs and subordinate directorates, changed the name to Direction des Services de Santé des Armées (DSSA) (Defence Medical Services Directorate).
